Chronic Disease Management

The HSE has established an Integrated Care Programme for the Prevention and Management of Chronic Disease which is free of charge for eligible patients.
This programme aims to provide better care to people with certain long-term illnesses. It allows for care to take place in a general practice setting and not in hospital outpatients.

If you qualify to participate, we will contact you to invite you to join. There are 2 visits a year included in programme.

The treatment programme is for patients aged 18+ who have a medical card, or GP visit card and have a diagnosis of one or more of the listed conditions below and it is free of charge.
• Type 2 diabetes
• Asthma
• Chronic obstructive pulmonary disease (COPD)
• Cardiovascular disease, including heart failure, heart attack (angina), stroke and irregular heartbeat (atrial fibrillation)
